As an associate faculty member you are eligible to participate in a DeltaCare dental HMO plan.

DELTACARE DENTAL HMO PLAN

Delta Care is a pre-paid dental program which requires the member to go to an in-network DeltaCare primary care dentist.  All care must be coordinated by the primary care dentist.  Many benefits are paid at 100%, with some benefits subject to co-payments.  Please refer to the benefit summary for additional plan coverage information and to Delta’s website at, www.deltadentalins.com, for the provider list.  Please note you must search the DeltaCare USA network.  Detailed instructions on how to find an in-network provider are attached.

Part Time-Faculty are eligible to enroll in the DeltaCare Dental HMO plan within 30 days of the start of Fall or Spring semester.  If you want to enroll for the Spring 2019 Semester you will need to submit your enrollment application no later than February 26, 2019.  If you submit your enrollment form & payment by February 10,  your coverage will begin effective March 1, 2019.  If you submit your enrollment form & payment between February 11 – February 26, your coverage will begin effective April, 2018.   If you miss the February 26^th deadline you will not be able to enroll until the Fall Semester.  If you are interested in enrolling, you are responsible for paying up front the premium from your first month of coverage through December 31, 2019.  Your cost will be the applicable rate listed below multiplied by the number of months you will be enrolled (10 months payment required up front for those who submit their forms by February 10^th, 9 months of payment for those who submit afterwards).    Please make your check payable to West Valley-Mission Community College District and send it with the enrollment application to my attention (West Valley-Mission Community College District, Human Resources – Tracey Frizzell, 14000 Fruitvale Avenue, Saratoga, CA  95070).  Also, please indicate the dental office you are selecting on the application – this can be done where it asks for Network Facility Name/Number.  If you do not choose an available primary care dentist, you will be auto-assigned based on your home address.

The monthly rates as of January 01, 2019 are as follows:

Employee:                                                                    $22.31/mo.

Employee plus 1 dependent:                                   $36.80/mo.

Employee plus 2 or more dependents:                  $56.84/mo.

Rates are subject to change during the policy renewal in January 2020.
