Submit your application to Tracey Frizzell in HR by the 1st Friday in April for Spring, 2019. Proof of health/dental/and or vision insurance must be submitted at least 3 weeks prior to the end of the semester. Each eligible member will receive a payment of either 1) the amount of premiums representing 6 months of coverage or 2) $2000, whichever is less. If the amount available for reimbursement of paid premiums ($80,000), is not sufficient to meet all the submitted claims, each eligible claimant shall receive an equal share of the $80,000.
