Fall 2020 SEMESTER HEALTH PREMIUM REIMBURSEMENT PROGRAM
Qualifying associate faculty members are eligible to receive up to $2,000 in reimbursement of medical, dental, vision, and prescription drug insurance premiums. Please note the plan covers premium costs for the qualifying associate faculty member only. This program does not reimburse for any claims costs (copays, coinsurance, etc.) or other family members who may be enrolled on the same plan as the qualifying associate faculty member. An application form is attached, and a detailed explanation of the program is included on the back of the application form.
Fall Semester Deadlines of Note
Friday, November 13th – Applications must be received by Human Resources on or before this date in order to be eligible for the program
Friday, December 18th – back up documentation showing proof of premium payment must be received by HR on or before this date
You can email, fax, or mail with a 11/13/20 postmark date for the application and a 12/18/20 postmark date for the payment documentation to meet all deadlines. You must make the first application deadline to be eligible for the program. My email, fax number, and mailing address are:
